Abbreviated Check List for DOT Drug Screen Collection

  1. 1. All supplies needed for collection are available and facility is ready and secure.
  2. 2. Donor is properly identified. Collector makes sure all information is present and correct on the Custody and Control Form (CCF).
  3. 3. Collector introduces self and briefly explains to donor the collection process. The collector explains that the only thing the donor can take into the toilet is a wallet. Coats and hats must also remain outside the toilet.
  4. 4. Donor is requested to empty pockets for inspection.
  5. 5. Donor is requested to wash hands.
  6. 6. Donor selects collection cup.
  7. 7. Donor goes into toilet for collection.
  8. 8. Donor gives specimen to collector.
  9. 9. Collector checks temperature (within 4 minutes of urination), appearance, and volume of specimen. Temperature should read between 90-100 degrees F. Volume must be at least 45 ml. Collector notes temperature check on CCF.
  10. 10. Donor is allowed to wash hands.
  11. 11. Specimen bottles are opened in presence of donor.
  12. 12. In presence of donor 15 ml of specimen is transferred into one of the bottles.
  13. 13. Remainder of specimen (at least 30 ml) is transferred into remaining bottle.
  14. 14. In presence of donor security seals are placed over tops of specimen bottles. Collector may date seals but donor MUST initial each seal.
  15. 15. Donor completes Pink copy of CCF.
  16. 16. Collector prints/signs name, notes date and time of collection and completes Chain of Custody on CCF.
  17. 17. Page 1 of CCF and split specimens are properly placed into security bag.
  18. 18. Donor is thanked and dismissed.
  19. 19. Remaining pages of CCF are appropriately distributed.
